The transition home is a challenging time as a new normal is discovered and accepted\, and can be particularly difficult for those with severe cognitive impairments or aphasia. This presentation will discuss the need for holistic and tailored education for those transitioning home\, and include practical clinical strategies to provide to patients and caregivers\, helping them to support ongoing success. \nObjectives: \nParticipants will be able to state challenges for survivors and caregivers during the transition from hospital to home \nParticipants will be able to state factors that impact a patient’s perception of success or failure during the transition from hospital to home \nParticipants will learn how to help facilitate a positive transition from hospital to home \nParticipants will learn ways to target functional goals for language and cognition in the home environment \n  \nAbout the Presenter: \nJessie Preston MA\, CCC-SLP\, BCS-S\nSpeech Pathologist at Rochester Regional Health and Clinical Educator at Nazareth College \nJessie Preston is a speech language pathologist with 14 years experience working with pediatrics and adults with a variety of diagnoses. Jessie previously worked in an acute rehabilitation unit specializing in treating individuals post brain injury and stroke. She currently provides home care services for both pediatrics and adults in addition to serving as a clinical educator for graduate level students in an on-campus clinic. Jessie is passionate about providing services for individuals with neurological impairments across the continuum of care. This presentation takes the second approach and consists of a detailed review of eight practices that will positively transform your life. \n  \nAbout the Presenter: \nGerry Brooks\, MA\, CCC\, CBIST\, CMTT\nBrain Injury Program Director at Northeast Center for Rehabilitation & Brain Injury \n\nGerry Brooks began his career in private practice in 1979 as a speech-language pathologist specializing in cognitive and communicative disorders. Since then he has logged thousands of treatment hours with survivors of brain injury and their families in both Ulster and Dutchess Counties. He has served as a consultant to hospitals\, rehabilitation centers\, and school systems and on the graduate faculty at SUNY New Paltz. \nIn 1998 Gerry joined the Northeast Center for Rehabilitation & Brain Injury’s planning team a year prior to its opening\, and he has been the Center’s Program Director ever since\, responsible for the development and oversight of the Center’s neurorehabilitation and neurobehavioral programs. Gerry has presented at local\, national and international venues. \nHe holds Certificates of Clinical Competence from the American Speech-Language-Hearing Association\, Brain Injury Specialist Trainer from the Brain Injury Association of America\, Technical Trainer from the Mandt System  of therapeutic behavioral support\, and Contemplative Care from the New York Zen Center for Contemplative Care. SUMMARY:Using a Bullet Journal to Promote Wellness for those Affected by Brain Injury: Professional Development Series
DESCRIPTION:  \nProgram Description: \nExecutive dysfunction is one of the hallmark deficits a person with a brain injury may experience\, making management of even the most basic tasks difficult. Bullet journaling is fast becoming one of the most popular ways to keep track of a variety of aspects of life. This course will discuss various cognitive deficits and how they can be managed by use of a bullet journal. A variety of sample journal pages and modifications will also be provided to aid in the establishing a basic bullet journal. \n  \nAbout the Presenter: \n\nLauren Holland\, MS\, CCC-SLP\, BCS-S\nRochester Regional Health \nLauren Holland\, MS CCC-SLP\, BCS-S has a bachelor’s degree from Nazareth College and a master’s degree in Speech Pathology from Boston University. She has spent her career working at the Golisano Restorative Neurology and Rehabilitation Unit\, an inpatient rehab\, specializing in stroke and traumatic brain injury for all ages. She also holds Board Certification in Swallowing and Swallowing Disorders. Dr. Seidel is also an assistant clinical professor of PM&R at Albert Einstein College of Medicine. Dr. Seidel did his PM&R residency at Thomas Jefferson University Medical Center in Philadelphia\, PA\, and his fellowship in Brain Injury Medicine at Kessler Institute for Rehabilitation in West Orange\, NJ. DESCRIPTION:  \n \nProgram Description: \nIn this webinar\, Dr. Leddy will discuss the physiology of concussion with respect to the autonomic nervous system and why exercise might help restore ANS control. He will then discuss the use of the Buffalo Concussion Treadmill Test (BCTT) for determining the level of exercise tolerance after a sports related concussion. Leddy is a practicing clinician\, active researcher and instructor at the University at Buffalo School of Medicine and Biomedical Sciences. He is the Medical Director of the University at Buffalo Concussion Clinic\, which is the first center in the U.S. to use a standardized exercise treadmill test to establish recovery from concussion and to use controlled exercise in the rehabilitation of patients acutely after concussion and in those with prolonged symptoms.
