SUMMARY:Transitioning Home: Considerations & Strategies to Support Ongoing Success: Professional Development Series
DESCRIPTION:  \nProgram Description: \nThe needs of people and caregivers following a brain injury extend beyond the walls of the hospital. The transition home is a challenging time as a new normal is discovered and accepted\, and can be particularly difficult for those with severe cognitive impairments or aphasia. This presentation will discuss the need for holistic and tailored education for those transitioning home\, and include practical clinical strategies to provide to patients and caregivers\, helping them to support ongoing success. \nObjectives: \nParticipants will be able to state challenges for survivors and caregivers during the transition from hospital to home \nParticipants will be able to state factors that impact a patient’s perception of success or failure during the transition from hospital to home \nParticipants will learn how to help facilitate a positive transition from hospital to home \nParticipants will learn ways to target functional goals for language and cognition in the home environment \n  \nAbout the Presenter: \nJessie Preston MA\, CCC-SLP\, BCS-S\nSpeech Pathologist at Rochester Regional Health and Clinical Educator at Nazareth College \nJessie Preston is a speech language pathologist with 14 years experience working with pediatrics and adults with a variety of diagnoses. Jessie previously worked in an acute rehabilitation unit specializing in treating individuals post brain injury and stroke. She currently provides home care services for both pediatrics and adults in addition to serving as a clinical educator for graduate level students in an on-campus clinic. Jessie is passionate about providing services for individuals with neurological impairments across the continuum of care.
