Description
The needs of individuals and their caregivers following a brain injury extend beyond the walls of the hospital. The transition home is a challenging and delicate time as a new normal is discovered and accepted. This transition can be particularly challenging for individuals with severe cognitive impairments or aphasia. The intricacies of acclimating back into home life can be difficult to predict for professionals who are unfamiliar with a patient’s environment and lifestyle. This presentation will include a discussion regarding the need for holistic and tailored education for individuals transitioning home. Participants will also learn practical clinical strategies to provide to patients and caregivers helping to support ongoing success.

Jessie is a speech pathologist for Rochester Regional Lifetime Care home care agency. As an adjunct faculty member at Nazareth college, Jessie is a clinical educator for graduate students in an adult neurogenic clinic.

Be able to state factors that impact a patient’s perception of success or failure during the transition from hospital to home
Learn how to help facilitate a positive transition from hospital to home
Learn ways to target functional goals for language and cognition in the home environment
